2) PAEDIATRICS - PAIN, MYOFASCIAL AND ORTHOPAEDIC

Many undergraduate courses don't include much paediatric content, and progressing straight to a full Diploma / PGCert programme can be a bit daunting - these emodules are a good starting point, to orient you to paediatric concepts

THERE ARE TWO OPTIONS FOR THIS TOPIC THIS YEAR:

SINGLE DAY:

saturday 29 JUNE 2019, AND

AS PART OF AN INTENSIVE BLOCK IN LANCASHIRE:

SUNDAY MAY 26 – TUESDAY MAY 28 2019

PAEDIATRICS – 3 DAYS. (still covers 30 hours)

Has separate emodule you can buy also.

Discusses inssues from the inside out!  Start with a recap on intrauterine isues, and the birth process.  Reminds us that birth is good, and undoes as much as it causes.  Looks at birth trauma and relates it via the developing autonomic nervous system and how myofascial pain impacts on autonomic state and steady balance / maturity.  Looks at infant settling, crying and sleep issues from this sense of arousal and hyper irritability on a stress myofascial person.  Considers how this might impact on sensory integration and threfore movement.  Reviews initial assessment routines.  Does not recap on the involntary mechanisms - this the the 'voluntary cranial course'!  Still relevant for those inclined to the OCF concepts though :-)

Don't forget this paeds series is about common conditions, it is not going to cover every pathological situation, but does recap on imporant red flags and gives you insights into the approaches for common presentations with infants. Evidence review, theory and practical on toy dolls (some videos too).

This lecture still starts out in the infant's first few hours and days, and now considers how breathing comes into play, how this has to mature, and how the complex interplay between suck-swallow-breathe can be interupted or distorted by stress and strain and myofsaial etc etnsions in the baby.  Considers the ongoing effects of the materno-foetal affective domain, and how the concept of the fourt trimester and the invisible umbilical cord still plays a role.  Looks in particular at some primitive relfexes, and assesment routines for the head, neck and face and well as the chest and mediastinum.  Challenges you to think if you really know where a baby diaphragm is!  Finally reviews the concept of periostium and its innervation.  Gives some assessment routines to consider. Evidence recap, theory and practical on toy babies (some videos too).

This day ges down to the nitty gritty of bones, myofascial structures and movement.   Goes from intra-uterine strain in a myofascial sesne,. looks at the imcremental advancedment of ossification and why here this occurs is interesting for practice.  Looks at head shapes - and how it is definately not all down to the birth presentation.  Considers torso, pelvis and overall movement patterns and continues the theme of sensory, proprioceptive, visual, auditory etc integration and how this impacts on musculoskeltal coordination and motor patterning development.  Discusses why that baby is not on its front more!  Gives a variety of assessment routines.  Evidence summary, theory and practical on toy babies (some videos too).

Finally this day gets into the more complex story of gait, hips and legs, and then torso integration, the changing body cavity shape as the child continues to move and grow, and sets up the story fo the second series of paeds lectures which is all about the visceral and systemic factors. This day focuses on orthopaedic factors, revisists the osseous strain story and looks at how motor patterning is still related to audotory, visual and 'social communication' integration.  You walk to talk, and this course explains how. Brings back into play the primitive reflexes and their integration. Gives a variety of assessment routines.   Evidence recap, theory and practical (might include hands on on participants, as some techniques can be done on the adults in the room!).  Some videos too.

Anterior structures and stomatognathic overview

The anterior fasicas and structures need careful exploration in many clinical syndromes and understanding these is vital if one is going to explore the more complex functions of the stomatognathic system such as breathing, speeech, swallowing and hearing / sight. Evidence recap, theory and practical.

Mediastinum, respiratory and diaphragm.

The root of the neck is buried in the mediastinum, your heart is always leaping up to your throat and mouth, and Littlejohn had intersting concepts about how the head and neck balanced onto the torso which has some foundation in current biomechanical concepts and frameworks based on visceral and vascular functions. The relationships also of the respiratory system and mediastinal fascias as they work they way through the thoracic inlet to the neck is also relevant to the global stomatognathic system, and must be explored if one is to udnerstand the thrat and face. This third day explores these issues. Evidence recap, theory and practical.

Larynx and speech.

The final day of this series looks at the voice and the functions of the larynx and how osteopaths can explore these delicate yet fantatically flexible functional units. The support of the breath from the diaphragm and below, the way airflow is controlled and the way the oral pharynx and the upper respiratory tract, mouth and lips all contribute to sound production can be explored with a variety of techniques. Laryngeal techniques are complex and not to be undertaken lightly as they can be stressful to the voice if done inappropiately, so it is important to take time with understanding this through the hands. Evidence recap, theory and practical.

When one wonders if osteopathy can help gynaecological function, that is a difficult thing to respond to, as it very much deoends what the pathological process is, what surgical, inflammatory or adhesion related factors are already in play, what levels of pain and central sensitisation of the CNS is involved - and that is just for starters.  So, before any 'treatment' can be given an indepth appreciation of the pathological effects of various gynae conditions must be reviewed.  This is the aim of the first day, and it brings in discussions about how to examine and assess these factors.

WEDNESDAY JULY 31 – FRIDAY 2 AUGUST 2019 ADVANCED WOMENS HEALTH – 3 DAYS

The second day focuses on the pelvic floor - which has a huge number of possible dsfunctions.  Prolapse is not all weakness, getting more (better?) tone in your pelvic floor in sometimes contraindicated.  It is necessary to be able to explore the perienum, levator hiatus and all layers of the pelvic floor from the outside, to aid local management, as well as prepare an understanding of how to address tehse factors intenrally if this is considered useful or necessary,  Coccygeal functions are also explored.  Issues around communication, consent and risk mangment are explored as well as body imagery, and emotional factors.  Many pelvic pain syndromes have complex behavioural and pain personality components and many patients need consdierably more careful handling when it comes to examinaing them than patients without complex histories concerning their most intimate selves. Evidence recap, theory and practical.  Contains in depth examination of the perineum and pelvic floor externally.

Embryology, location, anatomical variation, global links / core links

Has emodule you can buy separately.

WED-THURS-FRI

5-6-7 JUNE 2019, LANCASHIRE INTENSIVE(COVERS ALL 4 SESSIONS MATERIAL)

Put the tape measures away and learn how to really find and palpate the organs, and yes, its different to feeling bones, but you can still find your way around with a bit of guidance.

Learn embryology the fun way, and through that get a much better sense of the visceral 'ligaments' and realise that it is very artificial to discus minute techniques to very discrete structures - it doesn't reflect the reality of the structure and function of the body cavities and their contents.  This course doesn't include 'motility' as in an 'involuntary mechanism' it is very clear that motility mean peristalsis.

WED-THURS-FRI

5-6-7 JUNE 2019, LANCASHIRE INTENSIVE(COVERS ALL 4 SESSIONS MATERIAL)

This day is to get ones head around all the omentums, the paracolic gutters, the circulation and drainage of the organs etc and the basic physiology of the gastro-intestinal tract (from mouth to anus).   Orients you to a few myths in visceral work - like milking the gall bladder, reducing hernias and opening and closing the ileo-caecal valve.  There are caveats to all of these, and risks, benefits, plausibility and rational approaches are discussed in this review of the GIT and it location / assessment.

5-6-7 JUNE 2019, LANCASHIRE INTENSIVE(COVERS ALL 4 SESSIONS MATERIAL)

Getting into the chest, and understanding the mediastinum and its contents and their relationships is fascinating, and not nearly as difficult as one might imagine.  Come and learn how to make the rib cage 'more transparent' and how to access deeper structures and engage with different fascial receptors and dynamics in the chest.   This days reveals why the upper limbs and lungs are inter-related, and why the root of the neck is embedded deep into the chest - bring a cervico-brachial dynamic to mediastinal work in a more three-dimensional way.

WED-THURS-FRI

5-6-7 JUNE 2019, LANCASHIRE INTENSIVE(COVERS ALL 4 SESSIONS MATERIAL)

Understanding the interface behind and underneath the 'peritoneal balloon' helps us appreciate how the ventral lumbar column, with the aorta, vascular branches, plexi and cisterna are all in balance with the spine and the organ contents of the peritoneum.  Work in the retro-peritoneal space and the pelvis is at once simple (when approached in the right way) but at the same time appreciable as a highly sensitive and complex series of zones that needs more careful discrimination,  This day helps you get started.
